Your first visit will likely always be with of director of clinical operations, Dr. Trevor Eason. We highly recommend him and so do our practice members!
While Dr. Eason can act as your PCP, he is a specialist and will direct you accordingly until we can find proper placement with a long term PCP.

Insurance & Payment Options at Revive Health & Wellness
At Revive Health & Wellness, our priority is to provide exceptional, patient-centered care without the restrictions often imposed by insurance companies. While we do not file claims for office visits, you may utilize your insurance for essential medical services, including:
Lab Testing – Comprehensive diagnostic panels to identify underlying health concerns.

Unfortunately, many of the innovative and regenerative treatments we offer—such as hyperbaric therapy, IV infusions, and other advanced wellness services—are not covered by insurance. While we continue to advocate for expanded coverage, we remain committed to providing these therapies because we believe they are essential to achieving optimal health.

Have a 60-minute initial appointment with your provider(s) to discuss your health concerns and receive customized recommendations.
Have a 45-minute initial consultation with your health coach, nutritionist, and nursing team. They will work closely with your provider to create personalized lifestyle and nutrition strategies that fit your medical needs and daily routine. Your health coach will support you in implementing these changes every step of the way.

Certain costs are not covered under your membership and may require out-of-pocket expenses. However, insurance can and should be utilized when available, as it may cover some or all of these services.
Lab Tests: While many standard lab tests may be covered partially or fully by insurance, some may require a copay or may not be covered at all. Specialty tests, including stool, urine, and saliva testing, are not included in your membership are are uniquely needed on a patient-to-patient basis.
Non-Medical Services: Treatments and services such as IV infusion, Hyperbaric Oxygen Chamber, Injections, Regenerative Medicine, Chiropractic services (may be included), prescription and over-the-counter medications, and supplements are not covered under membership but may be recommended as part of your care plan.
HRT Membership Exception: Our HRT membership is unique in that it includes three lab panels annually, distinguishing it from our other membership plans.

Traditional healthcare testing, while adequate, is limited in scope and often incomplete, leaving many underlying health issues undetected. Using advanced specialty testing, we go beyond routine lab work to gain a deeper understanding of your health and create a personalized treatment plan tailored to your needs and long-term goals.
We guide you in selecting the right tests based on your health concerns, insurance coverage, and budget. During your initial consultation, your provider will work closely with you to explore your health goals and determine the most effective testing options. This may include comprehensive blood work, at-home specialty testing, or a combination of both. To ensure timely and accurate results, we recommend completing blood work at least two weeks before your next follow-up visit.

Yes, prescription medications are one of the most powerful tools in modern medicine and play an important role in Revive Health’s approach to care. When necessary, we prescribe medications as part of a comprehensive treatment plan to support healing and long-term wellness. However, we also recognize that many medications may be unnecessary or inappropriate for long-term use. Our goal is not only to use medications effectively but also to help patients safely transition off those that may no longer serve them.
When we prescribe medications, we take a holistic approach, understanding that nutrient depletions often occur as a result. Our team ensures that you receive the essential nutrients these medications may deplete, helping to maintain balance and prevent additional health concerns. Additionally, we develop a thorough plan to reduce or discontinue medications when appropriate, while simultaneously advising on lifestyle modifications, nutrition, and other natural interventions to support long-term health.
All of our medical providers—including doctors, nurse practitioners, and physician assistants—have been trained in conventional allopathic or osteopathic medical schools, PA schools, or nursing programs. They are fully licensed to practice medicine in their respective states and hold state DEA (Drug Enforcement Agency) licenses, allowing them to prescribe pharmaceutical medications.
While prescription drugs are a key component of our care when appropriate, Revive Health maintains the right and responsibility to choose if controlled substances or scheduled medications are appropriate and or inappropriate in specific patient presentations.
